Tinnitus is still a phenomenon with an unknown pathophysiology with
few therapeutic measures. During the last two decades, hyperbaric
oxygenation
therapy (HBO) has been used in the treatment of sudden deafness and
chronic
distressing tinnitus. In this study, we prescribed HBO to 20 patients
who
had had severe tinnitus for more than one year and who had already had
other forms of tinnitus therapy with unsatisfactory results. Four
patients
could not cope with the pressure gradient. The effect of HBO was
assessed
using subjective evaluation and VAS scores before and after HBO.
Follow-up
continued until one year after treatment. Six patients had a reduction
of tinnitus and accompanying symptoms, eight patients did not notice
any
change and two patients experienced an adverse effect. Any outcome
persisted
with minor changes until one year after treatment. HBO may contribute
to
the treatment of severe tinnitus, but the negative effect on tinnitus
should
be weighed
carefully.

In Laryngo-Rhino-Otologie, November 1998, wird im Rahmen der Otologie Podiumsdiskussion zur Hyperbaren Sauerstofftherapie folgendes ausgeführt:
"Die Meta-Analyse der Literatur (C. Heyden) und die ersten Resultate der vorgestellten Studie (Th.Lenartz, Hannover) können noch nicht als endgültiger Beweis dafür angesehen werden, daß die HBO der konventionellen Infusionsbehandlung überlegen ist bzw. sich mit ihr Behandlungserfolge erzielen lassen, die deutlich über den Plazeboeffekt hinausgehen. Es spricht aber einiges dafür, daß beim Hörsturz und beim Tinnitus nach erfolgloser Vorbehandlung eine Besserung der Beschwerden zu erzielen ist, sofern die Erkrankung nicht länger als drei Monate zurückliegt. Für Patienten, die diese Voraussetzugen erfüllen, soll die hyperbare Sauerstofftherapie empfohlen werden." (S.655)

In Laryngo Rhino Otologie, November 1998, there is an Otology panel discussion of HBO therapy about the following: " the Meta analysis of the literature (C Heyden) and the first results of the presented study (Th.Lenartz, Hanover) cannot be regarded yet as final proof for the fact that conventional HBO is superior or can success be obtained, which go clearly beyond the placebo effect. It speaks however some for the fact that with deafness and Tinnitus after unsuccessful pretreatment an improvement of the complaints is to be obtained, if the illness is not past longer than three months. For patients, who fulfill this requirement the hyperbaric oxygen therapy is to be recommended. " (S.655)
